Your ISW Facilitator Registry Form has been submitted successfully.

ISW Facilitator Registry Forms are not reviewed until we have received information from the FDW Trainer or Apprenticeship Facilitator Mentor that the Registrant has completed all requirements to be certified as an ISW Facilitator.

If we need additional information from you, we will contact you using the email you provided on the Registry Form.

We can only process Registry Forms on an occasional, small-batch basis. This is usually possible within 3 weeks of receiving a Registry Form. You will be sent an auto-message from the ISW website once your Registry Form has been accepted and you have been assigned an ISW website account. If you provided permission on the Registry Form, your name, affiliation and location will be added to the ISW Facilitator Registry web list at the same time.

If you have not received this auto-email three weeks after submission of the Form, please check your junk/spam folder for the email.

To reset your password and to access resources in the password-protected area of the website, follow the directions found on the FAQs page. You need to use the primary email you provided on this Registry Form in order to reset your password and to access your ISW Account.

If you need to change any part of your contact information, please do so by submitting your new contact information (Name, Affiliation, City, Province/State, Country, and email for ISW communications) on the Contact form on the main menu of the ISW website.
