If this was a multi-group FDW, complete a separate Form with information about the Trainer(s) and Participants for each FDW (i.e., complete one Form for each group of 4-5 participants.) If this was a combined FDW/TDW, you will also need to provide information about the Trainer-Trainee as indicated below.
